Prior to filling out the Pet Placement Form you must read this Pet Placement Agreement:
I do hereby agree to the following terms and conditions of Pet Placement:
1. That this animal is being adopted solely as a
household pet.
2. To treat my animal humanely throughout its life. This includes, but
is not limited to, providing adequate shelter from cold, heat and wet
weather, protection from fleas, ticks and mosquitoes, daily feeding,
constant access to fresh water and daily human attention.
3. To have my animal spayed/neutered, in accordance with the laws and/or
ordinances in force in the area in which you reside. If not already
done, in accordance with Florida Law within 30 days of this agreement if
the animal is 6 months of age or older or within 30 days after turning 6
months old. It is a misdemeanor to fail or refuse to comply with this
requirement and could subject you to a fine of up to $200.
4. To be responsible for all necessary booster shots as indicated below
and I further agree that the animal will receive full preventive and
necessary medical care in the future. This includes, but is not limited
to, distemper shots for dogs and cats, heartworm preventive for dogs,
feline leukemia shots for cats and rabies shots as required by law.
5. I agree not to sell, trade or dispose of said animal and shall not
allow the animal to be used for medical or any other experimental
purpose whatsoever.
6. Should I need to relinquish ownership of my animal, I will first
notify All Pet Rescue. If there is an All Pet Rescue foster home
available, All Pet Rescue will take the animal back. In the event there
are no available foster homes, upon prior approval, I may use All Pet
Rescue Mobile Pet Placements to place my pet. In order to do this, I must
provide current proof in writing from a veterinarian that the dog is
heartworm negative and a cat is feline leukemia/feline aids negative. Or
I may contact other animal welfare/humane organization for assistance.
7. That All Pet Rescue will not be responsible for the condition,
personality or health of the adopted animal and no warranty is implied
or intended.
8. That All Pet Rescue shall have the right to examine the animal after Pet Placement
and to investigate the conditions under which it is kept. I
agree that in the event of violation of any terms and conditions of this
agreement, All Pet Rescue may, without notice, repossess the animal
without recourse by me.
9. That if I am adopting a dog:
a) The dog will be kept inside a yard with
sufficient security to prevent the dog from escaping and/or I will keep
the dog inside.
b) At no time will I allow the dog to run
loose. I understand there are "Lease Laws" which state I must
keep my dog on a leash at all times when outside my fenced-in yard or
home.
c) I will comply with all laws requiring the
annual purchase of a rabies license tag and keep it securely fixed to
the dog's collar at all times, and other vaccinations that are needed.
d) I will not keep the dog chained outdoors,
garage or place in a cage, except for short periods of time.
10. That if I am adopting a cat:
a) The cat will be kept inside.
b) At no time will I allow the cat to run
loose.
c) I will comply with all laws requiring the
annual purchase of a rabies license tag and other vaccinations that are
needed.
11. That if I am adopting a bird:
a) The bird will be kept inside.
b) At not time will I allow the bird to fly
loose.
c) I will make sure that the bird has fresh
food, water and a clean cage at all times.
12. I further understand that any sum of money I have given to All Pet
Rescue is a donation toward its efforts in caring for this and other
animals and there are no refunds.
The Pet Placement fee covers medical expenses already incurred, therefore, it is not refundable.
Every effort is made to offer Pet Placements of only healthy, even-tempered animals. However, it is my responsibility as the new owner to choose carefully the animal I adopt. There are no warranties, expressed or implied, including but not limited to any implied warranties or merchantability or fitness of the animal adopted.
